Annual Consent Form

For us to offer support to young carers and for them to access, 1:1 mentoring, trips, activities, and clubs, we require a completed consent form from a parent or guardian. These forms are essential as they allow us to ensure the safety and wellbeing of young carers while they are in our care.

Please note that young carers will not be able to access, 1:1 mentoring, trips, activities, or clubs without a completed consent form.
