The Carer Navigator service supports friends and families who have a loved one admitted to Airedale General Hospital and Bradford Royal Infirmary. We have a team of staff, called Carer Navigators, who are dedicated to supporting people who are looking after a relative or friend in hospital.

You may not think of yourself as a carer, but you probably are. A carer is someone who provides unpaid help and support to a family member or friend who could not manage otherwise.

What Carer Navigators can offer:

We offer a range of information and support for carers. These include:

  • Help you be recognised as a carer and support you where you have concerns.
  • Support in meetings around discharge from hospital (we can attend meetings with you, if you feel you need us to be there).
  • Support with organising social and personal care.
  • Help with emergency planning.
  • Tell you about carer benefits and assessment entitlements.
  • Support with form filling and which services can help.
  • Support you the carer in group sessions.
  • Be someone the carer can talk to.
  • Connect you to other services, including Carers’ Resource services such as the Carer Card and Advice Line.
  • Let you know where you can get help after returning home.
  • A community-based worker can support you once the person you care for has been discharged from hospital.

Support for NHS staff and patients:

  • Do you have any concerns about their return home or how they or their carer will manage after leaving the hospital?
  • Could residential care be an option?

Carers Hospital Discharge Toolkit

The West Yorkshire Unpaid Carers programme has developed a West Yorkshire Carers Hospital Discharge toolkit and resources.

These new resources aim to improve unpaid carers’ experiences of discharge through increased identification, support, and signposting.

Carers’ Resource is supporting the rollout of the toolkit.
