Preventative work which is strength based and supports self care.  You could be working from the office, your own home & meeting carers in locations within local communities.

Role description:

Based in the Craven locality, with frequent travel across the district.

Hours of 26 per week with occasional Out of Hours phone cover required 

£21,817 (all salaries quoted for 37 hours)

   You will develop relationships with appropriate hospital and community teams and directly provide short-term support (no personal care) to clients in their own homes after discharge

Joining an experienced team, you will be confident in working flexibly and independently in the community and within hospital settings. Ideally with an understanding of the issues that can arise post-discharge, you will have:

  • Experience of working within a hospital environment will be an advantage
  • Confidence in liaising with hospital, community health & social care teams
  • Experience in proactively establishing referral pathways and promoting a service
  • Experience in assessing client needs and support planning
  • Experience in supporting people in their own homes
  • Excellent communication and people skills

Raising the profile of unpaid carers and reaching out to offer support, information and advice are the key dual roles of the Communications and Engagement Team.

You will work across the organisation identifying key communication and marketing routes to ensure that the voices of carers are heard locally, and nationally 

The successful candidate will be an exceptional communicator with the range of skills commensurate with the array of communication methods in use today.  Leading the Engagement Team, the post holder will ensure that all communication both externally and internally adheres to our branding guidelines and is presented in a high quality professional manner.

The successful candidate will be able to demonstrate the ability to write, co-ordinate and edit others contributions to our quarterly newsletter, manage relationships with the media, lead publicity and carer campaigns and further develop social media as an effective form of communication.
