Role description:

Carers’ Resource is looking for a new Trustee to join the board.

We are particularly keen to recruit a board member who has recent experience of being a young carer as this is an area we feel is under-represented by the current board.

Role description:

Home from Hospital Service Co-ordinator

York Hospitals & Selby District

Home based

With over 50% of your time expected to be spent at the hospitals, you will develop strong relationships with their staff.  Promoting the service across relevant wards and team meetings and generating appropriate referrals.

You will meet clients on the ward and also directly provide short-term practical help and support (no personal care) in their homes, within Selby District, following discharge.

Ideally with experience of working in or with a hospital team, you will have: –

  • Experience in developing relationships/service promotion
  • Confidence in liaising with  hospital staff and community health & social care teams
  • Experience in supporting people in their own homes
  • Ability to work flexibly and independently
  • Own transport essential
