Our Home from Hospital service in Bradford provides people with the help they need after leaving hospital – and is supported by a team of volunteers.

It also prevents readmissions as volunteers help people regain their confidence and independence.

It has been deemed a ‘lifeline’ by a grateful Bradford couple, who have told the Telegraph & Argus about their experience of the ‘godsend’ service.

A national report suggests almost 200,0000 people over the age of 75 have left hospital without proper support, increasing the likelihood of being readmitted.
