Carers’ Resource is excited to announce a new opportunity to make a real difference in the lives of vulnerable people. We are currently seeking a:
MAST Hospital and Community Support Worker
37 hours per week
Based at Airedale General Hospital
Salary: £24,437 – 26,802 (salary scale quoted for 37 hours per week.)
You will work as part of the MAST (Multi-Agency Support Team) in Airedale General Hospital providing specialist and specific support to individuals experiencing frailty who are frequent attenders at A&E, with the aim of reducing readmission and the frequency of presentation. You will provide support within the hospital setting to patients referred to the wider MAST Team and signpost into the community following discharge. You will also screen patients for alcohol use and mental health.
You will play a pivotal role in supporting individuals experiencing frailty enabling holistic person-centred decisions on the next steps for care and support, using guided conversations around advance planning and discharge planning,
Preferred working days are Monday to Friday. Some weekend working is required, currently 1 weekend day every 4 weeks.
