MAST is a partner project to support inpatients and patients that are being discharged from hospital that then need further support when at home. The purpose of the role is to ensure people receive ongoing support in the community to maintain their health, wellbeing and independence.
Candidates should have experience of health or social work, experience of working in a busy hospital environment and the ability to work 1 to 1 with individuals and to develop support plans for vulnerable people.
