We are delighted to be able to advertise for a Home From Hospital Team Leader.

You will be joining our experienced team working with people being discharged from hospital, supporting them to maintain their independence (no personal care).

You will have experience of working within hospital environments and experience of how hospital discharges are processed.

You will be confident in working flexibly and independently in the community. Ideally with an understanding of the issues that can arise post-discharge.

The Home from Hospital Team Leader will be based in Selby but will support the team in the development of the service across North Yorkshire. They will supervise on a daily basis the coordinators in Selby and support the whole team with enquiries and caseload pressures to meet the contractual needs of the service.
