Home from Hospital Coordinator
30 hours per week, Shipley office with extensive travel around Bradford District
£23,725 – £26,021 (FTE)
As a Home from Hospital Coordinator you will work with hospital discharge teams to identify appropriate referrals. You will assess clients at home and directly provide them with short-term practical help and support to reduce the risk of re-admission and assist in their return to independence.
You must drive and have access to your own vehicle.

Salary scale quoted for 37 hours per week.
New appointments are normally made to the bottom of scale.
