Home from Hospital Co-ordinator

30 hours per week, Shipley office with extensive travel around Bradford District

£23,725 – £26,021 (FTE)

You will be joining our experienced team working with people being discharged from hospital; carrying out weekly home visits and calls ,supporting them to maintain their independence.  You will be able to offer practical solutions and emotional support.

You must drive and have access to own vehicle.
