We are looking for an enthusiastic individual to take up the role of Community Carer Navigator.

The post holder will assist carers to navigate the health and social care system and support them to find the best possible solutions to their problems/better manage their caring responsibilities. You will work in partnership with other external services including community health teams and statutory partners.

The role includes identifying and addressing the needs of unpaid carers to promote, support and improve the mental, physical, emotional and economic well-being of the individual, so they can continue in their caring role. A strengths based, person centred approach is needed to empower individuals to identify, make and sustain positive lifestyle changes to improve their health and wellbeing.

You will be based in our Shipley office and work across the community of Bradford District.

Salary scale quoted for 37 hours per week.
New appointments are normally made to the bottom of scale.
