Carers’ Resource is delighted to be able to advertise this role to support carers. We are seeking a:
Carer Navigator – based at Bradford Royal Infirmary
£23,364 – salary stated for 37 hours
Hours of work 30 hrs (includes one weekend day)
You will support carers that are visiting the people they care for, whilst in the hospital. The role will focus on the provision of advocacy and representation of the carers’ needs whilst the patient is approaching discharge.
The role includes the provision of information, signposting, outreach, promoting self advocacy and completing wellbeing reviews which support carers to focus on their own wellbeing, helping them make good decisions. Preventative work which is strength-based and supports self care. The role includes one weekend day on shift across five days.
